WISD Requirements
Items Needed for 2017-2018 School Year:
The District is requiring all students to turn in a copy of the following items for the 2017-2018 school year.
El Distrito está requiriendo a todos los estudiantes que entreguen una copia de los siguientes documentos para el año escolar 2017-2018.
- Birth Certificate/Certificado de nacimiento

- Social Security Card/Tarjeta de numero social
|
- Immunization Card/Tarjeta de vacunas
|
- Identification Card of Guardian or person registering/Tarjeta de indificacion de guardián o persona registrando

- Proof of Income / Prueba de Ingresos: Pre-K & New to the district ONLY/Pre-k y Nuevo al districto Solamente
- Tax Return 2016 (the sheet that lists the student as dependent) or W-2 Form
- *Declaración de impuestos del 2016 o la forma W-2
- Current check-stub
- *talon de cheque reciente
- Current food stamps letter or SSI letter (parent can go by food stamps office and request a list of benefits)
- *Carta reciente de las estampillas o del SSI (pueden ir a la oficina de las estampillas y pedir la carta de beneficios)
- If parent does not have any of the above we have a form they can fill out. Si los padres no tienen ninguno de los documentos que están en la lista aquí en la escuela tenemos un documento que puede llenar.

- Proof of Residence / Prueba de Residencia
- Current Utility bill (Light, Water or Gas bill)
- *Factura de Utilidad reciente (Luz, Agua o Gas)
- Current Lease Agreement showing physical & mail address must be under the parent or guardian name (current rent receipt must state the address on receipt)
- *Contrato de arrendamiento con dirección física y de envío reciente (recibo de renta reciente con la dirección en el recibo)
- Current Bills: Car Insurance, Public assistance: food stamps, child support, TNAF, social Security, trash disposal, credit card, etc. NO PHONE BILLS
- *Facturas recientes: Seguro de Auto, Asistencia Pública: estampillas, manutención de hijos, TNAF, seguro social, de la basura, de tarjeta de crédito, etc. NO FACTURAS DE TELEFONO
- If parent or guardian does not have any of the above items and are residing with relatives or friends. Parent must fill out a notarized form and can be picked up at F.D.R.
- Si los padres o guardianes no tienen ninguno de los documentos mencionados y residen con familiares o amigos, los padres deben llenar una forma notariada y puede ser levantada en F.D.R.
